Dear Camp Joey, Camp Quest and Odyssy Parents,
I would like to take this opportunity to welcome you to our 2009 summer camp program. Summer is quickly approaching and it is time to complete the paperwork so that your child may attend camp. These are Department of Public Health regulations and must be strictly followed. Failure to adhere to these regulations will mean that your child CANNOT attend camp. Here are the important due dates that you must be aware of:
a. Due ASAP
b. The health form is due March 2nd for all weeks of camp. If we do not receive your child’s health form they will not be able to attend camp for the summer of 2009. Each health form must be filled out completely and have attached a copy of the child’s immunizations with dates and signed by a physician.
a. Due the Thursday before your child is to begin camp (by 4:30 pm)
b. The Authorization to Administer Medication to a Camper Form must be filled out completely, signed and turned into the Recreation Office by the Thursday before the camper is to begin camp. If we do not receive the form by Thursday at 4:30 we will be unable to administer medication to that child for the entire week of camp.
a. Due the first morning of camp. (Would prefer them be sent into the office earlier)
b. The Parent Packet must be filled out completely, signed and given to the Recreation Office or Camp Director before the child can be left at camp. The Parent Packet includes information about camp as well as a pick up policy, sunscreen policy, and a special request form.
4. Field Trip Permission Forms
Please print & complete the field trip permission form that you have registered your child for. Field Trip permission forms are due by the first day of your camp session.
Forms can be found online or be picked up at the Recreation Office Monday – Friday 8:30 am – 4:30 pm.
