Passports (rev 6/18/07)
Please refer to US Department of State Passport information for the latest requirements for travel.  The Western Hemisphere Travel Initiative, to require passports to travel to locations where formerly only a birth certificate was required,  has been temporarily postponed due to high volume of passport requests.

Travel to Puerto Rico does not require a passport.

PASSPORTS - are valid for all travel between the US and foreign destinations.  Adult passports are valid for 10 years, minor passports are valid for 5 years.

PASSPORT CARDS - Passport Cards are valid for international travel by LAND or SEA only between the US and Canada, Mexico, the Caribbean and Bermuda.  This is a less expensive option for travelers whose trips will be limited to these destinations and will not travel by air.  The same forms are used to apply for a Passport Card.  SEE  travel.state.gov for additional details on Passport Cards.

Travelers should allow 4-6 weeks for standard passport processing and 2-3 weeks for expedited processing.  Information on how to apply for a passport, including how to expedite processing, is available at http://travel.state.gov. Renewal applicants requesting expedited service must write "EXPEDITE" on the outside of the envelope containing their application.

Applications are accepted Monday-Friday 8:15 am - 4:45 pm.  Appointments are preferred and can be made by calling the Town Clerk's Office at 781-431-1019 ext. 2252.

To file a passport, you will need:

Citizenship Evidence
        
» Current or previous passport

or

» Certified Birth Certificate containing:
Full name of applicant
Date of birth
Place of birth
Certification (Authorized seal)
Date of original record/registration date
Registrar's signature

or

» Certificate of Naturalization

NOTE: For children under 17, please obtain additional information from the U.S. Department of State's Passport Services website at  http://travel.state.gov/passport/get/minors/minors_834.html

Children under 17 require BOTH parents’ signatures, or notarized written consent from absent parent, evidence of sole custody, death certificate, or written explanation why absent parent’s consent cannot be obtained.

Primary (Good Photo likeness required):
        
» Valid U.S. Drivers License
        
» Pilots License, State or Federal work ID card
        
» Current Student ID card (school or college)
        
» Military ID (Dependent ID)
        
» Passport (Foreign or U.S.)/Naturalization Certificate, Certificate of Citizenship
        
» INS Alien Registration Card (Green Card)/Employment authorization


Secondary (Submit up to FIVE of the below list):
        
» State ID cards (additional items must be submitted to support ID cards)
        
» Credit Cards, Medical Cards, Insurance Documents, Pay Stubs, Utility Bills,
Welfare/Benefit Cards, Leasing documents, Mortgage documents
        
» Marriage Certificate/Divorce Decree, Income Tax Records
        
» School Records, Yearbook with photo, Diploma/Degree, Technical School
certificate, GED certification, Vehicle Registration/Title, Military records (DD214, honorable Discharge)

2" x 2" full face, plain light background on acceptable photo paper.

Appropriate Fees
        
» Age 16 and over: $75 check or money order payable to the U.S. Department of State PLUS $25 to the Town of Wellesley (may be cash or check)
        
» Age under 16: $60 check or money order payable to the U.S. Department of State PLUS $25 to the Town of Wellesley (may be cash or check)

» Expedite fee: An additional $60 fee to the U.S. Department of State is required when the passport is required in 2-3 weeks.

Travelers needing emergency passports within two weeks must contact the Boston Passport Agency directly.  1-877-487-2778.
